Mail-in Enrolment Form
Due to the closure of the ServiceHub in-person services, Mail-in Enrolment Forms will not be accepted by mail or fax. Enrolment Forms with credit card information will not be accepted by email.
The G. Raymond Chang School of Continuing Education Certificate Program Registration/Transfer (via RAMSS)
Login to my.ryerson.ca in your RAMSS Student Centre. No registration form is required. Enter the pre-approval code (if necessary).
Program Time Span Extension Form
Certificate requirements must be successfully completed within your official time span from the time you were first admitted into the program; refer to Time Span in our Glossary of Terms. Should you require additional time to meet your program requirements, complete this form, have it approved by your program director, and submit it to Student Records for processing.
Certificate Program Withdrawal Form
Should you decide not to continue in your Chang School certificate program, complete this form and submit it either in person, by mail, or by fax to Student Records.
Program Pre-Approval Forms
You must complete an application for pre-approval into the following programs. Once you receive your pre-approval code, you can register in the certificate through Certificate Program Registration/Transfer (via RAMSS).
